L-Carnitine TMAO: Why Gut Bacteria Matter for a Capsule Ingredient
L-carnitine TMAO is a gut chemistry link: bacteria in the gut convert L-carnitine into TMAO, a compound that has been tied to atherosclerosis. A 2022 trial of 2 g a day for six months raised total and LDL cholesterol. Synaptril lists L-carnitine without an amount, and our label estimate puts it far below 2 g.

Definition01
What is TMAO and how does L-carnitine make it?
TMAO is a compound made when gut bacteria break down certain nutrients, including L-carnitine, and the liver finishes the job. Higher levels have been linked to atherosclerosis, the buildup of plaque in arteries (NIH ODS, Carnitine).
Carnitine is found in red meat and dairy, so most people already make some TMAO from food. A supplement adds to that supply.
Cooking and gut bacteria both shape the result. Two people eating the same meal can end up with different TMAO levels, which is why researchers study groups and not single cases.
A link is not a verdict. Studies tie TMAO to heart risk, but they do not prove that a small carnitine capsule causes heart disease in a healthy adult.
The trial02
Does L-carnitine raise cholesterol?
L-carnitine raised cholesterol in one 2022 trial. People who took 2 g a day for six months had higher total and LDL cholesterol (NIH ODS, Carnitine).
That is a high dose by supplement-label standards. At about 3 g a day, people also report nausea, diarrhea and a fishy body odor, which is linked to the same gut conversion.
| Dose per day | What was reported |
|---|---|
| 2 g for 6 months | Higher total and LDL cholesterol |
| About 3 g | Nausea, diarrhea, fishy odor |
| Synaptril ceiling, 144 mg | No study at this dose |
| Synaptril estimate, at most about 58 mg | No study at this dose |
The Synaptril figures are our estimates from the label order rules (21 CFR 101.36, supplement labeling), not lab results.
Scale03
Is the L-carnitine in Synaptril a TMAO concern?
The L-carnitine in Synaptril is probably a small TMAO concern, because the estimated amount is a small fraction of the 2 g tested. Even our upper ceiling of 144 mg is about 7% of 2 g.
For scale, one capsule a day for 30 days at the 144 mg ceiling adds up to about 4.3 g across the whole bottle. The 2 g trial dose was daily, for six months, which is about 360 g in total. The difference is large.
But the label prints no amount, and no trial covers the capsule. "Probably small" is a reading of the label, not a safety finding.
People with heart disease, high cholesterol or kidney disease should ask a physician before using any carnitine product. Keep taking prescribed medicine, and tell the physician about every supplement.

How can you use carnitine more carefully?
Use carnitine more carefully by knowing the amount, the form and your own risk. Three steps cover most of it.

FAQ05
Carnitine and TMAO questions
Does everyone make TMAO from carnitine?
Gut bacteria differ between people, so the amount varies. Diet shapes the bacteria too, and people who eat little meat tend to convert less. Because of that spread, a single dose does not predict your level.
Is TMAO only a concern for people with heart disease?
TMAO has been linked to artery plaque in general, so it matters for anyone with heart risk factors. It is not a diagnosis tool. Ask a physician about your own heart risk, not a supplement label.
Do other Synaptril ingredients affect TMAO?
The ingredient sources we cite tie TMAO to carnitine, not to magnesium, alpha-lipoic acid, turmeric or coenzyme Q10 (NIH ODS, Carnitine). That is a statement about the evidence we found, not a claim that the others have no effect.
Should I stop L-carnitine if I am worried about TMAO?
Ask a physician first, and do not change prescribed medicine on your own. Stopping a supplement is easy and low risk for most people. A physician can weigh your heart and cholesterol history.
Does a fishy smell mean TMAO is high?
A fishy body odor is a known effect at about 3 g a day of carnitine (NIH ODS, Carnitine). It points to the same gut chemistry. If you notice it at a low dose, stop the product and tell a physician.
